RPM services often go hand-in-hand with Chronic Care Management (CCM) services. CCM services consist of establishing, implementing, revising, or monitoring a care plan for a patient with multiple chronic conditions. See below for specific codes.
EDUCATION
Onboarding a new patient into an RPM program, equipment set-up and patient education on the equipment.
CPT Code 99453: Remote monitoring of physiologic parameter(s) (e.g., weight, blood pressure, pulse oximetry, respiratory flow rate, glucose), Onetime initial; set-up and patient education on use of equipment.
SUPPLIES
Reported when the patient receives a RPM device for monitoring for each 30-day period.
CPT Code 99454: Remote monitoring of physiologic parameter(s) (e.g., weight, blood pressure, pulse oximetry, respiratory flow rate, glucose), initial; device(s) supply with daily recording(s) or programmed alert(s) transmission, each 30 days
DATA REVIEW
Reported when a qualified healthcare professional spends 20 minutes of time monitoring, including interactive communication via phone, text, or email.
CPT Code 99457: Remote physiologic monitoring treatment management services, 20 minutes or more of clinical staff/physician/other qualified healthcare professional time in a calendar month requiring interactive communication with the patient/caregiver during the month.
EXTENDED DATA REVIEW
Reported when an additional 20 minutes of interactive communication is provided to patient/caregiver and is billed in conjunction with CPT code 99457.
CPT Code 99458: Remote physiologic monitoring treatment management services, clinical staff/physician/other qualified health care professional time in a calendar month requiring interactive communication with the patient/caregiver during the month; additional 20 minutes
