We offer a variety of ways to work with Keto-Mojo to get blood glucose and ketone monitoring kits into the hands of your patients.
Step 1: Patient Receives Device (CPT 99454)
You may use the Remote Patient Monitoring (RPM) supplies CPT code: 99454 to bill when the patient receives an RPM device.
Step 2: HCP Shows Patient How to Use Device (CPT 99453)
Once patients own their meters, healthcare practitioners can show them how to use the meter during an office or remote visit (billable under RPM education CPT code: 99453).
Step 3: Collect, Evaluate and Interpret Patient Data (CPT 99091)
HCP spends a minimum of 30 minutes collecting and interpreting the data stored digitally and/or transmitted by the patient/caregiver.
Step 4: Review Data with Patient (CPT 99457)
From that point forward, you can review patient data remotely using your MyMojoHealth practitioner account, with our EHR integration option, via integration with your proprietary app using our API or SDK, or the patient may choose to provide you with a CSV file for review. You can bill for RPM data review using CPT code 99457 (20 minutes of time monitoring, including interactive communication via phone, text, or email).
Optional: Extend Review with Patient (CPT 99458)
You can bill for RPM data review using CPT code 99458 (extended data review, reported when an additional 20 minutes of interactive communication is provided to patient/caregiver and is billed in conjunction with CPT code 99457).
Reported when the patient receives a RPM device for monitoring for each 30-day period.
Code 99454: Remote monitoring of physiologic parameter(s) (e.g., weight, blood pressure, pulse oximetry, respiratory flow rate, glucose), initial; device(s) supply with daily recording(s) or programmed alert(s) transmission, each 30 days.
Onboarding a new patient into an RPM program, equipment setup and patient education on the equipment.
Code 99453: Remote monitoring of physiologic parameter(s) (e.g., weight, blood pressure, pulse oximetry, respiratory flow rate, glucose), One time initial set-up and patient education on use of equipment.
Reported when a qualified healthcare professional spends at least 30 minutes collecting, evaluating and interpreting the data.
Code 99091: Collection and interpretation of physiologic data digitally stored and/or transmitted by the patient/caregiver to the physician or other qualified health care professional, requiring a minimum of 30 minutes of time, each 30 days.
Reported when a qualified healthcare professional spends 20 minutes of time monitoring, including interactive communication via phone, text, or email.
Code 99457: Remote physiologic monitoring treatment management services, 20 minutes or more of clinical staff/physician/other qualified healthcare professional time in a calendar month requiring interactive communication with the patient/caregiver during the month.
Reported when an additional 20 minutes of interactive communication is provided to patient/caregiver and is billed in conjunction with CPT code 99457.
